Application to use animals for undergraduate/ school student project

This form is for application to use animals for student project, such as honours project, Undergraduate Research Opportunities Programme in Science (UROPS), Science Research Programme (SRP), Science Mentorship Programmes (SMP) and others.

This form is required even if the student project constitutes part of an approve protocol, as it is important for IACUC to review how the students will be involved in animal use.

This application form comprises a main form and separate forms for procedures.
Submit the Main Form and the applicable Procedure Forms only. Also submit Attachment C if you would like to share animal tissues with other researchers.
