Annual Protocol Review

All animal use protocols are to be reviewed by IACUC annually. The Annual Protocol Review (APR) forms listed below should be completed for IACUC review every year. They also serve as final reports upon completion of the projects / workshops.

This form is for annual review of protocols for research projects including student projects and biodiversity/field studies. It is to be reviewed annually on the anniversary of the protocol approval date. Completed forms should be submitted 1 month before the due date.

This form is for annual protocol review of protocols for classroom teaching/workshop. It is to be reviewed annually on the anniversary of the protocol approval date. Completed forms should be submitted 1 month before the due date.

This form is for the annual review for protocols for a breeding colony. It is due annually in January and should be submitted by the 5th, irrespective of the approval date.
